Grade 9 Assitant Referee Re-certification Meeting

Due to the large numbers of Referees, we are unable to make changes to this schedule. We appreciate your cooperation and apologize for any
inconvenience.

There will be a brief Review Session at the start of each session so please be on time!

All AR's Are Subject To Re-Take The Written Test So....STUDY Your Course Materials!

BRING YOUR USSF Form!! It MUST Be Brought With You To THIS MEETING!!

To the BEST OF YOUR ABILITY, Complete The Following Sections:

Section 1: Name, Address, Phone, Email Address, Date of Birth

Section 5: Check the appropriate YES/NO Box AND SIGN AND DATE (VERY
IMPORTANT)

For other items on the Form that you are unable to complete, LISRA Personnel will be available to help...however, to keep things moving,
PLEASE COMPLETE as much as you can in Section 1.

Bring a CHECK For $30.00 made payable to ENY (Eastern NY Soccer). This is your yearly USSF Re-certification Fee.

You CANNOT be re-certified without making this USSF payment, which is collected by ENY.

AR Coordinators....PLEASE be sure these instructions are passed on to ALL of your Grade 9 Assistant Referees as we are not able to communicate
directly with them at this time.
